That’s serious. Imagine someone working hard on an investigation only for “human error” to destroy everything. It’s things like this that make people lose faith in the systems. I’ve seen similar nonsense happen here too, one small mistake and the case is just as good as dead. They need to put better checks in place to stop this kind of foolishness in our system. Not every mistake should be excused with “human error”.
